For the past several years, I had sinus infection after sinus infection.  I was sick all the time.  I finally decided to see an ENT doctor in January 2007.  They performed a CT scan of my sinuses to see where the problem was and then started treatment - a round of very strong antibiotics, a round of steroid pills, and a steroid nasal spray.  After everything else was gone, I would remain on the nasal spray to keep the sinuses open and hopefully prevent the infections from recurring.  So far so good!  I think this is the longest I've been infection-free in a number of years.  The spray is expensive, but the benefit is worth it.
